Qualifications for Home Care


Criteria that Qualifies for Home Care
Suffers from severe shortness of breath or shortness of breath on exertion. Patient that may show symptoms of residual weakness. Needs assistance for all activities. Requires assistance to ambulate. Is unable to safely leave home unassisted. Shows signs of confusion, inability to go out of home alone. Dependence on adaptive device (cane, walker, or wheelchair).
Patients Who May Need Referred for Home Care
If a patient is unstable when walking or moving. If medications are frequently changed. Patient has frequent hospital admissions. Patient has no consistent caregiver. Patient has questionable comprehension related to medication. Non-compliance with medication and diet. Often misses scheduled doctor's appointments. Physician has not seen patient for extended period of time
